How response types work in Audit Form Templates

Each question in an Audit Form Template has a response type that determines how the auditor answers it. When adding or editing a question, use the response type dropdown to choose the format that best fits the information you need to collect.

Available response types in Audit Form Templates

The following response types are available:

  • Yes / No / N/A: The auditor selects one of three options. Use this for compliance checks where a finding is either present, absent or not applicable.
  • Single / Multiple Choice: The auditor selects one or more options from a list you define. Use this when you need a controlled set of possible answers.
  • Text: The auditor enters a free-text response. Use this for questions that require a description, observation or explanation.
  • Location: The auditor enters or captures a location. Use this when you need to record where a specific finding occurred.
  • Date: The auditor selects a date. Use this for questions that record when something was last done, inspected or is due.
  • Signature: The auditor provides a signature. Use this to confirm that the audit or a specific section has been reviewed or agreed by a named person.
