How to add a risk matrix to a document

You can rate a risk inside any document, on a matrix that matches your own
scoring. It's how you score the hazards in a risk assessment, a policy or a
procedure, and the rating is saved with the document.
How to add a risk matrix
- Go to 'Documents' in the left sidebar and open your document.
- Type '/' and choose 'Risk matrix', or select 'Risk matrix' from 'Insert' in the toolbar.
- Click the cell where your likelihood meets your impact. Each cell shows its score and its risk band, and your selected levels and score appear below the grid.
How to match the matrix to your scoring
The matrix starts as a five by five grid, with impact running from
'Insignificant' to 'Severe' and likelihood from 'Rare' to 'Almost certain'. You
can change all of it.
- Click the gear icon below the matrix to open 'Configure Risk Matrix Size and Labels'.
- Under 'Matrix Size', set 'Rows' and 'Columns'. Any size from three to seven works.
- Under 'Impact Labels' and 'Likelihood Labels', rename each level to the wording your organisation uses.
- Save your changes.
Note: Scores are banded and colour-coded as Very low, Low, Medium, High or
Very high. Use four or more rows and columns to see all five bands, as smaller
grids combine them.
What happens next
The matrix is part of the document, so it's covered by everything else Documents
gives you: every change is tracked in version history, you can set a review
date, publish it when it's ready, and ask named people to acknowledge that
they've read it.
