How to add a risk matrix to a Document
Applies to user roles: Account Owner, Manager, User
Available on platforms: Vatix Workspace web app
You can rate each hazard on a risk matrix set up to match your organisation's own
scoring, so it's clear at a glance which risks need attention first. The matrix is
part of the Document, so its ratings are saved in every version you publish.
A risk matrix scores a risk by where its likelihood meets its impact, and colours
each score by its band, from Very low to Very high.
How to add a risk matrix
- Go to 'Documents' in the left sidebar and open your Document.
- Type '/' and select 'Risk Matrix', or select 'Risk matrix' from 'Insert' in the toolbar.
- Click the cell where your likelihood meets your impact. A new matrix starts with the middle cell selected, so change it to your own rating. Each cell shows its score and its risk band, and your selected levels and score appear below the grid.
You can add a risk matrix for each rating you need. To set out the hazards, who
might be harmed and the controls around them, see
How to write a risk assessment in Documents.
How to match the matrix to your scoring
The matrix starts as a five by five grid, with impact running from
'Insignificant' to 'Severe' and likelihood from 'Rare' to 'Almost certain'. You
can change all of it.
- Click the gear icon below the matrix to open 'Configure Risk Matrix Size and Labels'.
- Under 'Matrix Size', set 'Rows' and 'Columns'. Any size from three to seven works.
- Under 'Impact Labels' and 'Likelihood Labels', rename each level to the wording your organisation uses.
- Click 'Save Configuration'.
Note: Scores are banded and colour-coded as Very low, Low, Medium, High or
Very high. Use four or more rows and columns to see all five bands, as smaller
grids combine them.
What happens next
The matrix is part of the Document, so it's saved in each version you publish and
included when you ask people to acknowledge the Document.
